What determines whether it's a gesture made with racist intent is the context, and when it was made. Obama used an 'OK' sign during a speech in 2017, before the gesture's reassignment at the hands of 4chan had gone widely public. Its use by avowed white supremacists is now common.
R31, part of what distinguishes Santos' gesture as racist is the effort he takes to make it covert.

R33, he held his hand below, as if attempting to hide it. He is a far right Republican, and the use of the gesture to mean 'white power' is not inconsistent with that.
